GUIDELINES FOR MEDICAL, SURGICAL, DENTAL MISSION:

1. Our medical, dental & surgical mission will be at the SJA campus on April 2,
2012, Monday 8AM-2PM.
2. All participants of dental, operation tuli & Pap smear must pre-register at your barangay. Each barangay is entitled to have five participants for each procedure. We will issue a stub to each participant upon registration in your barangay and this stub must be presented to the pregistration table/table 1 on the day of the mission for confirmation in our master list. Last day of registration is March 31, 2012, 12:00 NOON. The registration lists will be collected from the barangays on March 31, 2012, 1pm onwards.
3. All those who want to participate in our medical & pediatric mission (adults & children) must come on the day of the mission for registration on a first come, first serve basis.
4. There will be 300+ free blood glucose tests for the adults.
5. There will be free reading glasses to be given away.
6. All participants must arrive at the St. Joseph Academy campus on or before 8AM
7. For pre-registered patients (dental, operation tuli & Pap smear), please proceed to the “Preregistration Table/Table 1.”
8. Medical & Pediatric patients please proceed to the “Registration Table/Table 2.”
9. An usher or usherette will guide you to the assigned room for the procedure.
10. A guardian or parent who will sign the consent prior to the procedure must accompany all participants of operation tuli and dental mission who are 17 years old or younger.
11. For the participants of Pap smear, there should be:
• No bleeding/spotting
• No sexual contact for three days(3) days prior to pap smear
• No abnormal discharge
• If the participant has any of these three symptoms she is advised to give her slot to another qualified participant
12. For the participants of dental mission, please
• Take a bath & brush your teeth before coming to the mission
13. For the participants of operation tuli & pap smear, please
• Take a bath before coming to the mission
